A common occurrence that results in short root fills are due to ledging the canal.  The trick in managing these cases is determining whether the root fill is short because the canal joins a second canal, as is common in lower molar distal canals; or may be due to a practitioner not respecting the root canal curvature with correct filing techniques or using inflexible large sized files down canal curvatures.
